Several years ago, the Committee on Undergraduate Education
developed sample Teaching Feedback Questionnaires to be used by faculty
sometime after their classes are underway. Faculty who have used these
questionnaires have found them to be helpful in assessing how their
teaching is coming across to students. In contrast to the Course Evaluation
Forms, which are distributed at the end of the semester, the Teaching
Feedback Questionnaires are designed solely for your use to gather specific
information about your teaching at a time when adjustments can be made.
You are urged to make an occasion during the middle of the semester
to solicit responses from your students about your teaching. Two
versions of a questionnaire are available, Version
A with open-ended and Version
B with multiple-choice questions. An order
form is also available for requesting forms in quantities
needed for your classes. Alternatively, you may want to adapt
one of the questionnaires or even develop your own to serve your
particular needs.
The questionnaires are for your use only. Students should be instructed
to return them directly to you.
