Pre-Departure Forms and Student Handbook

Program Agreement and Limits of Liability Waiver: Download
Read and sign. Also requires signature by parent/legal guardian if you are less than 18 years of age.

Medical Forms: Download

  • Health Information Form: Fill out and sign. The physician who completes your Physician’s Statement must also sign this form.
  • Physician’s Statement: Physician signs.
  • Medical Treatment Authorization: Read and sign.

Emergency Contact Information Form: Click here to complete this form online.

Proof of health insurance:  Send us a copy of your health insurance card or policy (you are responsible for verifying that your insurance covers you properly while abroad).

International Student Identity Card (ISIC): Get this card and send us a copy of the card with your photo on it. This card provides access to a network of discounts and benefits. We are requiring you to get this card in order to assure that each participant will have minimum emergency coverage abroad. The ISIC card will provide you with basic accident, sickness and other travel-related coverage outside the U.S. Visit www.myisic.com to learn more and to purchase the card.

***If you are not able to obtain the card prior to the Penn Summer Abroad documents deadline, you may submit a copy of the receipt as proof of purchase instead of the card.***

Pre-Departure Basics

Passport

If you do not already have a passport, make sure you apply for one right away. Passport application forms and instructions can be obtained at the U.S. Post Office or at http://travel.state.gov. If you already have a passport, check to be sure the passport will be valid for the entire time you will be outside of the U.S.

Visa

Most countries where Penn Summer Abroad has programs do not require U.S. citizens to have visas for short-term stay. If a visa is required for your location, you will receive further information from the program. If you are not a U.S. citizen, please contact the embassy of the countries you will be visiting to see if a visa is necessary. If so, you will need to apply for it several months in advance.

If you are planning to travel to countries other than the site of your program, also check requirements with the consulates of these specific countries.

Air Tickets

Penn Summer Abroad does not organize travel overseas. Start researching flights early. There are many possibilities and a large variation in prices. You will receive the list of all students in your program. Contact the ones who are likely to travel from the same airport to see if you may be able to travel together.

We recommend purchasing travel insurance that will provide coverage for lost baggage or unforeseen events that may require rescheduling your ticket.

Immunizations

Make sure you receive all appropriate inoculations. Consult with your physician. Any of the following sites are good resources for information on required or recommended vaccines and immunizations for travel abroad.
